The difficulty of conceiving a child is a problem particularly felt by the couples who confront it, with important repercussions also from a psychological point of view.

To achieve the "goal" of having a baby, it is essential that both men and women maintain good fertility; the causes of infertility, in fact, in terms of frequency are distributed almost equally between the two sexes.

For the male sex, it must be remembered that the spermatozoa must, relative to their size, face a journey of length not indifferent because it must be able to go up all the female reproductive system to reach the fallopian tubes, where it will fertilize the egg.

A visit to the urologist andrologist and a seminal fluid test would often be enough to recognize and treat the male ailments that make them sterile. If there was a specialist andrologist/urologist in each assisted reproduction center, access to the centers, the costs of infertility treatment could be reduced and the results improved by at least 30%. 25 to 30% of those under the age of 18 may already have an infertility problem due to a decidedly irresponsible lifestyle.

With a simple visit it is possible to detect pathologies such as varicocele, which can harm fertility but which can also be easily treated if they are recognized in time. Men are unaware of sexually transmitted diseases and drugs that influence male fertility. Compared to women who take more initiative against an infertility problem, men are more careless in the face of red flags.

According to the World Health Organization, male infertility is "the inability to impregnate after at least 12 months of regular sexual activity and not protected by any contraceptive method, with a normally fertile partner". It is distinguished by:

• primary infertility, when the man has never had a pregnancy

• secondary infertility when the man has already caused pregnancy before.

For specialists (endocrinologists and, in particular, uro-andrologists) who find themselves investigating problems of male infertility, it is very important to collect a good medical history: the patient should inform the doctor of

• any problem with the genital system (inflammation of the prostate, sexually transmitted diseases and difficulties in sexual activity)

• past infections (mumps and tuberculosis),

• genetic diseases (cystic fibrosis),

• systemic diseases (diabetes)

• possible exposure to chemical and physical agents (drugs, toxic, heavy metals and high temperatures) that can compromise human reproductive health

• Another important cause of dysfunction is represented by varicocele, the varicose dilation of the pampiniform venous plexus of the testicle.

A recent study investigated how the morphology of the sperm head is also changed and these are unable to get inside the oocytes. It also contributes, in many cases, to the lack of integrity of the DNA contained in the head of the sperm, to their fragmentation, true cut of the double helix, which causes many cases of failure of medically assisted procreation, despite excellent quality embryos.

The DNA damage is due to the increase in free radicals present in the seminal fluid, is linked to diseases such as varicocele, dilation of the testicular veins with blood reflux with an increase in temperature which affects the sperm formation. It also contributes to cryptorchidism, downward failure in the birth of one or both testicles in the scrotum. The cocktail of smoke, alcohol and drugs contributes to destroying the DNA of the sperm head, as well as obesity and the consumption of anabolics, which are increasingly appreciated by minors.

The physical examination includes palpation of the testicles (in particular to assess their volume) and rectal exploration for the evaluation of the prostate.

Obviously, to objectively assess the fertility of a male subject, it is compulsory to resort to the examination of the seminal fluid (spermiogram associated with spermioculture). Reproductive difficulties can be linked to sperm abnormalities:

• low counts (oligospermia)

• reduced motility (asthenospermia)

• abnormal form (teratospermia)

• or the coexistence of the three aforementioned conditions (oligo-astheno-teratospermia).

Azoospermia, on the other hand, is the complete absence of sperm in the sample analyzed.

Another essential laboratory test is the measurement of hormone levels (prolactin, luteinizing hormone, follicle-stimulating hormone, testosterone, estradiol and thyroid hormones): infertility can, in fact, be a consequence of endocrine problems, affecting in particular sex hormones, but also, sometimes, concerning thyroid function. The level of the follicle-stimulating hormone is very important: if it is high, it indicates a primary testicular lesion, while if it is low, in conjunction with significant spermatic abnormalities, it suggests hypogonadotropic hypogonadism, it that is to say a poor testicular function in turn determined by insufficient hormonal secretion by the glands which control gonadal activity (the hypothalamus and the pituitary gland). Another dysfunction of the endocrine axis is constituted by hyperprolactinemia (high levels of prolactin decrease the production of testosterone, the male hormone par excellence).

All diagnostic investigations are supplemented by scrotal doppler ultrasound and genetic analysis, in order to look for chromosomal abnormalities.

Orange juice and grapefruit for breakfast. For lunch, however, a tomato salad garnished with basil or enriched with green leafy vegetables, such as arugula. Among the foods you should never miss: cabbage, broccoli, spinach and legumes (increase sperm production), nuts (make them more active and mobile) and also saffron, liver, chicken and fish, which improve their quality and prevent defects.

To be avoided, however, sugary drinks, excess red meat, fried foods, snacks, and foods high in saturated fat. Again, the Mediterranean diet is an elixir of health.
