Brief overview of what spermatozoa are:

Spermatozoa are the male gametes, that is, the cells that, in humans, are responsible for reproductive function, once they meet female cells with the same job (eggs).

Produced by the testes (male gonads), during so-called spermatogenesis, sperm are very small cellular elements (5-7 micrometers, the diameter, and 70 micrometers maximum, the length) and with a structure very particular, which includes:

• An intermediate portion, rich in mitochondria;

• A tail, equipped with mobility and called flagellum.

What is teratozoospermia?

Teratozoospermia is the name for sperm alteration, characterized by the presence of a large number of malformed spermatozoa.

Teratozoospermia is a condition of considerable interest in men who wish to have children, because a large number of poorly formed sperm are associated with a decrease in male fertility (male infertility).

WHO definition of teratozoospermia

According to what was established by the World Health Organization (WHO) in 2010, a man suffers from teratozoospermia when, in the sperm produced following a classic ejaculation, the spermatozoa of normal morphology are lower than 4%.

Translated, this means that:

• A man with at least 4% of sperm with a normal morphology cannot be considered to be affected by teratozoospermia;

• A man, in whose ejaculation the percentage of malformed sperm is greater than 96%, is a carrier of teratozoospermia.

Curiously, before 2010, the WHO was of another opinion on teratozoospermia and believed, on the basis of medical knowledge of the time, that a male individual suffered from the aforementioned sperm alteration when, in the ejaculate, the percentage of spermatozoa of normal morphology, it was less than 14%.

Causes

In most cases, teratozoospermia depends on unknown and unidentifiable causes; in other circumstances, however, its trigger is recognizable and may consist of:

• Cancer treatments such as chemotherapy or radiotherapy;

• Use of drugs that interfere with testosterone or GnRH (gonadotropin releasing hormone) and, therefore, with the production of morphologically normal sperm;

• A genital infection, i.e. an infection of one of these organs in which the semen and seminal fluid (prostate, seminal vesicles, epididymis and urethra) flow regularly;

• Testicular trauma or testicular disease, such as testicular cancer or hydrocele;

• Varicocele: It is a pathological alteration of the testicular (or spermatic) veins, as a result of which the latter appear dilated and one or both testicles are swollen;

• Meningitis;

• Diabetes mellitus;

• A severe febrile state or the consequences of a severe febrile state;

• Abuse of alcohol or illegal drugs (eg hashish, marijuana, etc.);

• Smoke.

Symptoms and complications

The characteristics of sperm, observed by spermiogram, include:

• Sperm viscosity,

• Liquefaction of sperm,

• Sperm volume,

• The number and morphology of sperm e

• Vitality and motility of sperm.

Once the presence of teratozoospermia has been noted, the next step is to look for the causes responsible for the altered morphology of the spermatozoa; this research may require the performance of various diagnostic tests, including instrumental tests.

Basic rules for obtaining a reliable spermogram

For the result of a spermogram to be reliable, compliance with certain rules preparatory to the aforementioned laboratory examination is fundamental; in more detail, these preparatory rules are as follows:

• Complete abstinence from sexual activity within 3-5 days before the examination;

• Ejaculation obtained exclusively by masturbation;

• Before masturbation, ensure adequate hygiene of the hands and penis;

• Provide for the collection of all the sperm produced (therefore including the one for which the penis must be pressed, after the main emissions);

• Recover in a clean place;

• For collection, use a suitable, sterile container (eg urine containers);

• Hermetically close the container used to avoid accidental loss of the collected semen sample;

• After collecting the semen sample, send it to the testing laboratory as soon as possible (within 30 to 60 minutes), avoiding thermal shock. Simplifying everything is the increasingly common possibility of collecting the sperm sample directly in a bathroom in the analysis laboratory;

• Inform the personnel of the health laboratory of the analysis of any therapy or illness followed or occurred during the last three months;

• Inform the health laboratory staff of the analysis of any breach of any of the above preparatory rules, in order to understand whether or not it is necessary to repeat the procedure for taking semen samples.

What should normal sperm look like?

A sperm is morphologically normal when:

• The head is oval, it measures between 5 and 10 micrometers long, and its main components (cell nucleus and acrosome) are clearly distinguished from each other;

• The intermediate part is linear, opposite the acrosome and at least one and a half times longer than the head;

• The tail is the thinnest part of the entire male gamete, is no less than 50 micrometers long and is mobile.
